All of us here at Heart Pharmacy strive to put your safety and comfort first. For peace of mind, it is often a relief for those living independently to know there is someone caring for their well-being and ensuring medications are being taken correctly every day. If you or your loved one would benefit from having daily oversight in taking medications, insulin injections, blood glucose monitoring or need an extra hand with eye drops or medicated patch application - we have a dedicated team that will come in to the home. These visits also help to increase social interaction in an increasingly isolated world.
We believe very strongly in the power of a team-based approach to care. All services are overseen by Clinical Pharmacists who connect with a patient’s family and Family Doctor when needed.
Heart at Home offers in home medication administration to our clients with a dedicated group of Licensed Practical Nurses, Registered Nurses, and Home Care Aides.
We offer our clients daily assistance with:
Taking your daily medications
Eyedrop and Eardrop Administration
Medicated Patch Application
Insulin Injections and Blood Glucose Monitoring
Blood Pressure Monitoring
Inhaler Administration
Daily wellness check in
Companionship
For patients taking three or more medications, the medication assistance program is typically covered through provincial or personal health plans, subject to the patients’ deductible.
This is an affordable way to access introductory home care services without a 3-hour visit minimum.
